PAYMENT
SCHEDULE
This
form will register your interest in the selected
class with CIMSS. To complete registration,
please send $100 Registration Fee for each registration
to: CIMSS, P.O. Box 673, Sonoma Ca 95476.
Registration
Fee is non-refundable after the first class.
Fees for books, supplies and student insurance
are due the first day of class. Payment of tuition
is possible paid in full the first day, in two
installments beginning with the first day of
class and completing by the midpoint of class
or by prior
agreement with Kate.

have any questions. We accept cash, check, Visa/Mastercard
or you
may apply for the Sallie Mae 'The Career Training
Loan' program. Forms are available at the school.
